Security & Privacy Training

Computer Security Basic Awareness Training (Required)

[Training is required and should be completed per instructions provided during employee orientation. NOTE: You will need the free Adobe Reader (link will open in a new window) software in order to complete the training using the materials referenced below.]

Per DHHS policy all Division employees, contractors, temporary staff, volunteers, interns, guests, and other individuals who have or will be provided access to DPH information systems must follow the instructions below to complete proper Computer Security Basic Awareness Training:

Instructions
Requirement 1 (Two parts):
  1. Review the Computer Security Basic Awareness Training presentation (PDF, 149 kb).
  2. Print and sign the DPH Computer Security Basic Awareness Training Record (PDF, 15 kb) in order to confirm training.

Requirement 2 (Two parts):

  1. Review the Acceptable Use for DHHS Information Systems Policy (PDF, 48 kb).
  2. Print and sign the User Certification of Notification and Agreement of Computer Use Policy (PDF, 16 kb).

Requirement 3 (One part):

Please return both signed forms to DPH Human Resources.

Basic Privacy Awareness Training - HIPAA: How DHHS Privacy Policies AFFECT YOU (Required)

Requirement 1 (Two parts):
  1. Review the DPH Basic Privacy Training presentation (PDF, 284 kb).
  2. Print and sign the DPH Privacy Training Record (PDF, 9 kb) in order to confirm training.

Requirement 2 (Two parts):

  1. Review the Division of Public Health Confidentiality Agreement (Word document, 31 kb)
  2. Save, print, and sign the Division of Public Health Confidentiality Agreement (signature line is at bottom of Word document linked directly above)

Requirement 3 (One part):

Please return both signed forms to DPH Human Resources.
